[3 min 45 sec] Southern Regional AHEC's Family Medicine Center (FMC) and Information Access Center (IAC) have partnered to help heart patients take charge of their health through information. A patient's doctor may prescribe some or all of four learning modules: diet, monitoring, medications, and lifestyle changes. The librarians in the IAC will help patients access the learning modules, and they will show patients how to access their own FMC electronic medical records.
